To register, you must provide the following documentation:

-- Certified Birth Certificate 

-- Social Security Card

-- Medicaid card, Wellcare card, EBT Card (all that apply)

-- Proof of Residency (i.e. phone bill, water bill, etc.)

-- Completed Registration Form - available at the school

The following documents must be received by the school within 30 days after school starts:

-- Ear, Eye, and Dental Certificate (Form 3300)

-- Immunization Certificate (Form 3231)

You will be contacted if any additional documentation is needed.

At the beginning of school your child will be tested for speech and other services.  You will be contacted, if your child need any services.
